Job Description
I am seeking an experienced web developer to create an online form tailored to the needs of our cosmetic clinic. It’s crucial that the form is user-friendly, secure, and integrates smoothly with our existing website. Here's what we need:
Key Components:
– **Treatment Options**: Include checkboxes for Botox, Dermal Fillers, Bio Stimulator Injections, Hyalase, and PDO Threads. Each option should allow users to explore more information along with possible adverse reactions and side effects
**Information Collection**: For each treatment selected, the form should prompt users list current medications and medical history, indicate if they've received tetanus or whooping cough vaccines in the last 6 weeks, and state if they are pregnant or breastfeeding.
– **Contact Information**: The form must include fields for patients to provide their name, phone number, Date of birth, address and email address securely.
-**Medication Script**: The form must include a field that is a script with name of medication/ dosage/ treatment area, with side effects and adverse reaction. Then a place for Patient to sign electronically and also place for doctor to sign electronically.
Ideal Skills and Experience:
– **Web Development & Design**: You should have a strong background in creating interactive and secure web forms, preferably with experience in the healthcare or cosmetic industry.
– **Security**: Implementing data protection measures to ensure patient confidentiality is essential.
– **User Experience Design**: The form should be intuitive and easy to navigate for a diverse range of users.
– **Integration Skills**: Ability to seamlessly integrate the form into our existing website, with a focus on maintaining site performance and aesthetics.
